Georgia works differently from other states. Under Structured Family Caregiving, the paid caregiver must be a family member related by blood, marriage, or adoption who lives in the same home as the person receiving care, and caregiving is their primary role. A spouse, legal guardian, or conservator cannot be the paid caregiver, and friends or neighbors are not eligible. Caregivers receive a tax-free daily stipend through Georgia Medicaid rather than an hourly wage.

Program Guide
Structured Family Caregiving (SFC) is a Georgia Medicaid service under the Elderly and Disabled Waiver, delivered through the Community Care Services Program (CCSP) and SOURCE and overseen by the Georgia Department of Community Health. It supports a family caregiver who lives with the person receiving care: the caregiver receives a daily stipend plus real backup — an approved SFC agency provides health coaches, a registered nurse, and required training.
For families already providing round-the-clock care for a parent or relative at home, SFC recognizes and compensates work they are already doing.
The waiver serves Georgians age 65 and older, and people with physical disabilities under 65, who meet a nursing-facility level of care and have (or qualify for) Georgia Medicaid — Alzheimer's and other dementias count as qualifying physical conditions. SFC itself is for participants assessed to need substantial daily hands-on support, with a caregiver living in the same home.
Screening starts with a Determination of Need-Revised (DON-R) score through your Area Agency on Aging, followed by an in-home assessment and physician certification. Medicare alone does not qualify, and financial eligibility is determined by the Medicaid program. Waiver capacity can be limited, so there may be a waiting list prioritized by need.
Georgia's 12 Area Agencies on Aging are the single point of entry — families start through the statewide Aging and Disability Resource Connection. Final Medicaid financial eligibility is determined by the state.

SFC is a live-in family model: the caregiver must be related to the participant by blood or marriage and share the home — most often an adult child, but a sibling, grandchild, or other relative can qualify.
By waiver rule, the participant's spouse, legal guardian, or representative payee cannot be the paid SFC caregiver, and a parent cannot be paid for a minor child. Friends and neighbors are not eligible under SFC. Caregivers must be at least 18 and legally able to work in the U.S. Eligibility for any caregiver is determined by the program during enrollment.
