Program Guide
Community HealthChoices (CHC) is Pennsylvania's Medicaid managed-care program for long-term services and supports, run by the Pennsylvania Department of Human Services through its Office of Long-Term Living. Instead of paying for care service-by-service, the state contracts with CHC health plans that coordinate home care, personal assistance, and other supports for eligible Pennsylvanians.
For families, the practical meaning is simple: if your loved one qualifies, Medicaid — not your family — pays for approved in-home care, and the participant has a real say in who provides it.
CHC serves adults age 21 or older who are enrolled in Medicaid and either have both Medicare and Medicaid ("dual eligible") or need long-term services at a nursing-facility level of care. The person receiving care must live in Pennsylvania, and Medicare alone does not qualify.
Functional eligibility is confirmed through the state's Functional Eligibility Determination (FED) assessment, arranged through Pennsylvania's Independent Enrollment Broker, along with a physician certification. Financial eligibility is determined by the Medicaid program — not by us, and not by any home care agency.
Pennsylvania accepts Medicaid long-term-care applications online through COMPASS, by phone, or at a county assistance office, and the statewide Independent Enrollment Broker coordinates the CHC waiver enrollment steps — the assessment, the paperwork, and plan selection.
That process has real steps, and this is where families most often stall. CHC includes participant-directed options: the participant can act as the employer of their direct care worker, and Pennsylvania also offers a budget-authority model called Services My Way. A fiscal agent handles payroll and taxes, so being the "employer" does not mean running payroll yourself.
Family members can provide paid personal assistance under these options — an adult child, sibling, other relative, friend, or neighbor. By program rule, the participant's spouse cannot be the paid caregiver, and neither can their legal guardian, power of attorney, or representative payee. Caregivers must be at least 18 and legally able to work in the U.S. Eligibility for any caregiver is determined by the program during enrollment.
